Umrah is the 'lesser pilgrimage' to the Ka'bah, which can be performed at any time of the year. Its essential acts are Ihram, Tawaf, Sa'i and shaving or trimming the hair. The Prophet ﷺ said: 'One Umrah to the next is an expiation for what is between them.'
1
Prepare for Ihram
Before reaching the Miqat (for travellers from Pakistan by air, usually Yalamlam, announced on the plane), clip nails, remove unwanted hair, take a ghusl (or wudu) and apply perfume to the body (not the ihram sheets). Men wear two unstitched white sheets (izar and rida) and sandals that leave the ankle bone uncovered. Women wear their normal modest clothing, without face veil (niqab) or gloves.
Many Pakistani pilgrims change into ihram at home or at the airport and make the intention later near the Miqat. Crossing the Miqat without ihram requires a penalty (dam).
Sahih al-Bukhari 1524; Sahih al-Bukhari 1539

Two rak'ahs and Niyyah
If it is not a disliked time, pray two rak'ahs nafl with the head covered. Then uncover the head (men) and make the intention for Umrah. The words below are commonly taught; the intention in the heart is what counts.
Allahumma inni uridul-'umrata fa-yassirha li wa taqabbalha minni.
Meaning: O Allah, I intend to perform Umrah, so make it easy for me and accept it from me.
The Prophet ﷺ said 'Labbayka 'umratan' when entering ihram for Umrah; the longer wording is from fiqh books.
Sahih Muslim 1232
3
Talbiyah
Immediately after the intention, recite the Talbiyah: men aloud, women quietly. Saying it (in the Hanafi school, with the intention) puts you in the state of ihram. Repeat it often until you begin Tawaf.
Meaning: Here I am, O Allah, here I am. Here I am, You have no partner, here I am. Surely all praise, grace and sovereignty belong to You. You have no partner.
Sahih al-Bukhari 1549; Sahih Muslim 1184
4
Prohibitions of Ihram
While in ihram do not: cut hair or nails; use perfume or scented soap; have marital relations or intimate talk; hunt; argue or fight. Men must not wear stitched clothing shaped to the body, or cover the head; women must not cover the face with a veil touching it, or wear gloves.
Violating a prohibition may require a penalty (dam or sadaqah); ask a scholar if this happens.
Qur'an 2:197; Sahih al-Bukhari 1542; Sahih al-Bukhari 1838

Entering Masjid al-Haram
Enter with the right foot, reciting the dua for entering the mosque. When you first see the Ka'bah, make dua, as it is a time of acceptance.
Bismillahi was-salatu was-salamu 'ala Rasulillah. Allahummaftah li abwaba rahmatik.
Meaning: In the name of Allah, and peace and blessings be upon the Messenger of Allah. O Allah, open for me the doors of Your mercy.
Sahih Muslim 713; Sunan Ibn Majah 771

Tawaf (7 circuits)
Stop the Talbiyah. Men uncover the right shoulder (idtiba'). Stand in line with the Black Stone (Hajar al-Aswad, marked by a green light), make the intention for Tawaf, raise your hands and say 'Bismillahi Allahu Akbar', then kiss the Stone, touch it, or simply point to it from a distance (istilam). Walk anticlockwise keeping the Ka'bah on your left, including the Hatim. Men walk briskly (ramal) in the first three circuits. Each circuit ends at the Black Stone; repeat the istilam at the start of each. Seven circuits complete the Tawaf. You must have wudu.
Bismillahi, Allahu Akbar.
Meaning: In the name of Allah; Allah is the Greatest.
There are no fixed duas for each circuit; recite any dhikr, dua or Qur'an in any language. Do not push or harm others to reach the Black Stone. Ramal and idtiba' apply only in a Tawaf followed by Sa'i.
Sahih al-Bukhari 1603; Sahih al-Bukhari 1613; Sunan Abi Dawud 1883

Between Rukn Yamani and the Black Stone
When you reach the Yemeni Corner (Rukn Yamani), touch it with the right hand if easy, without kissing or pointing. Between it and the Black Stone, recite this Qur'anic dua.
Rabbana atina fid-dunya hasanatan wa fil-akhirati hasanatan wa qina 'adhaban-nar.
Meaning: Our Lord, give us good in this world and good in the Hereafter, and protect us from the punishment of the Fire.
Qur'an 2:201; Sunan Abi Dawud 1892

Two rak'ahs at Maqam Ibrahim
Cover the right shoulder again. Pray two rak'ahs behind Maqam Ibrahim if possible (or anywhere in the mosque if crowded), reciting Surah al-Kafirun in the first and al-Ikhlas in the second. Avoid the disliked times.
Wattakhidhu min maqami Ibrahima musalla.
Meaning: And take the standing place of Ibrahim as a place of prayer.
These two rak'ahs are wajib in the Hanafi school and Sunnah in the others.
Qur'an 2:125; Sahih Muslim 1218

Drink Zamzam
Drink Zamzam water to your fill, standing facing the Qiblah if possible, and make dua for anything you need. The Prophet ﷺ said: 'The water of Zamzam is for whatever it is drunk for.' If possible, return to touch the Black Stone before Sa'i.
Allahumma inni as'aluka 'ilman nafi'an, wa rizqan wasi'an, wa shifa'an min kulli da'.
Meaning: O Allah, I ask You for beneficial knowledge, abundant provision and a cure from every illness.
This dua is reported from Ibn 'Abbas (RA); any dua may be made.
Sunan Ibn Majah 3062; Sunan ad-Daraqutni (dua of Ibn 'Abbas)

Sa'i between Safa and Marwah
Go to Safa and recite the verse below, then face the Ka'bah, raise the hands in dua and say the dhikr of Tawhid. Walk to Marwah (one lap); men jog lightly between the green lights. At Marwah, face the Ka'bah and repeat the dhikr and dua. Marwah back to Safa is the second lap. Complete seven laps, ending at Marwah. Wudu is recommended but not required.
Innas-Safa wal-Marwata min sha'a'irillah. Abda'u bima bada'allahu bih.
Allahu akbar (3x).
Meaning: Indeed Safa and Marwah are among the symbols of Allah. I begin with what Allah began with.
Allah is the Greatest. There is no god but Allah alone, without partner; His is the dominion and His is the praise, and He has power over all things. There is no god but Allah alone; He fulfilled His promise, helped His servant and alone defeated the confederates.
The verse is recited only at the start of the first lap. Between the hills, 'Rabbighfir warham, innaka antal-a'azzul-akram' is reported from Ibn Mas'ud and Ibn 'Umar (RA).
Qur'an 2:158; Sahih Muslim 1218

Halq or Taqsir (end of Umrah)
Men shave the whole head (halq, which is better) or trim the hair (taqsir) from all over the head. Women gather their hair and cut about a fingertip's length (about an inch) from the ends. Your Umrah is now complete and the ihram restrictions are lifted.
Hanafi minimum: at least a quarter of the head trimmed by a fingertip's length. Shafi'is consider three hairs sufficient; Malikis and Hanbalis require trimming from all over the head. The Prophet ﷺ prayed three times for those who shave.
